The purpose of the 15 min. discovery call is to provide you with an opportunity to ask questions and allow us both to assess goodness-of-fit in potentially working together.  

I do not accept insurance, however, I'm happy to provide you with a super bill that has the information needed to submit to your insurance should you desire to attempt to submit for partial reimbursement for services provided by an out-of-network dietitian.  Prior to working together, it is advised that you call your insurance to learn what, if any, reimbursement you can expect to receive from your insurance for working with an out-of-network dietitian.  

Finally, my licenses allow me to work with individuals via Telehealth in the following states:  AK, AZ, CA, CO, CT, HI, ID, IN, MD, MA, MI, NH, NY, OK, OR, PA, TX, UT, VT, VA, WA, WV, WI, WY

If you do not currently reside in one of the above states, I'm unfortunately unable to work with you at this time. However, I'm happy to provide you with referrals to other trusted providers if you send me an email (katesdaleyeats@gmail.com).
